Early Childhood & Family Support

The Community Health Services Division also supports our community through our Children and Youth with Special Health Care Needs (CYSHCN) program, our Early Support for Infants and Toddlers (ESIT) program, our Supplemental Nutrition and Assistance Program (SNAP-Ed), and the San Juan Early Learning Coalition.

Children and Youth with Special Health  Care Needs (CYSHCN)

Children with special health care needs are children from birth to 18 years of age (up to 21 years of age for certain diagnoses) who have (or are at increased risk of) serious chronic physical, developmental, behavioral, or emotional conditions. We offer public health care coordination and referral services to families of CYSHCN in San Juan County regardless of income. Early Support for Infants and Toddlers (ESIT)

Home visits, assessments, information, support, and referrals for families of children from birth to age three (3) years who have known or suspected developmental delays or disabilities. Learn more about the ESIT program or refer children to the Toddler Learning Center program.

Supplemental Nutrition and Assistance Program (SNAP-Ed)

SNAP-Ed can help people learn how to make healthy food choices within a limited budget and choose an active lifestyle. SNAP-Ed works with partners to make the healthy choice the easy choice. The San Juan County Early Learning Coalition is a member of the regional Northwest Early Learning Coalition with the mission of making excellence in early learning achievable and sustainable for every child in San Juan County.
