Children and Youth with Special Health Care NeedsChildren with special health care needs are children from birth to 18 years of age (up to 21 years of age for certain diagnoses) who have (or are at increased risk of) serious chronic physical, developmental, behavioral or emotional conditions. We offer Public Health services to families in San Juan County regardless of income. Learn more about State and Local outreach and resources.
Early Support for Infants and Toddlers (ESIT)
Provide home visits, assessment, information, support and referrals for families of children from birth to age three years who have known or suspected developmental delays or disabilities. Learn more about the ESIT program. Refer children to our program.
SafeCare Parenting Education Program
Home visiting program for families of children ages six years and under to provide intensive parenting and health education and support.
Oral Health & Access to Baby and Child Dentistry (ABCD) Program
The Access to Baby and Child Dentistry program — ABCD — puts young children on a lifelong path to good oral health. ABCD connects low-income families with dentists who know how to care for young kids, preventing tooth decay early and educating parents about how to take good care of their children’s teeth. TB & Pregnancy Testing
Provide confidential tuberculosis and pregnancy testing, and referrals to providers for follow up care.
Communicable Disease Surveillance
San Juan County Health & Community Services relies on healthcare providers, laboratories, hospitals and the general public to report certain communicable diseases to us. Reporting communicable diseases helps protect our community, and assists our efforts around disease prevention, surveillance, and outbreak investigation. Supplemental Nutrition and Assistance Program (SNAP) Ed
